What is Green Prescription?
Green Prescription (GRx) is a free health promotion programme which aims to support you to make positive health changes to your lifestyle.
GRx aims to improve the health and wellbeing of adults through physical activity and nutrition support. Through education, removal of barriers, and development of personal skills GRx aims to enhance physical, mental and social well-being.
Once referred by your doctor or nurse, our Health Team at Sport Taranaki will contact you and get you underway with a suitable activity programme. We’ll provide the support, advice and resources needed to help reduce those barriers and get you started on a journey to a healthier lifestyle.
GRx involves phone support and/or face-to-face consultations with you to find the best solution and support for your circumstances.
Who can access a Green Prescription?
Green Prescription is a prevention programme prioritising referrals for inactive adults who are ready to make changes towards a healthier lifestyle. You need to be medically stable and able to engage in low to moderate activity.
How can I get a Green Prescription?
Ask your doctor or practice nurse about Green Prescription. Your doctor may refer you to the Green Prescription team if you both agree that your health would benefit from becoming more active.
Alternatively you can refer yourself by contacting the Green Prescription team at Sport Taranaki by phoning 0800 ACTIVE (22 84 83) or filling out a SELF REFERRAL FORM FOR GREEN PRESCRIPTION
Consent and Agreement Terms
By agreeing to proceed with your Green Prescription, you consent to Sport Taranaki sharing information with the Taranaki District Health Board and the Ministry of Health. Information shared will be for statistical purposes only and individuals will not be identifiable. Sport Taranaki will comply with the Privacy Act 1993, Health Information Privacy Code 1994 and Health (Retention of Health Information) Regulation 1996.
